Community-Oriented Primary Care (COPC) is the provision of primary care services to a defined community, coupled with efforts to identify and address the major health problems of the community. The COPC process consists of: defining and characterizing the community; identifying the community’s health issues; modifying or designing health programs; and monitoring or evaluating the effectiveness of programs.

COPC projects generally fall into one of four categories:

  • Illness/Disease in Rural Communities
  • Health Education/Health Promotion
  • Health Care Utilization and Access
  • Environment and Organization of the Community
